Step 2:
Kumpletuhin ang kinakailangang detalye ng iyong claim sa FPG Insurance claims notification page. Siguraduhing kumpleto ang lahat ng impormasyon at documentong nagpapatunay sa iyong claim.
(Claim notification must be must be given 15 days from time of accident)
Claim requirements: (please submit complete claims document within 60 days from time of accident)
A. General Requirements
- Duly Accomplished Personal Accident Claim Notification Form A & B (Required)
- Accident Report (in the absence of accomplished PA Claim Notification Form)
- Police/Investigation Report (for Vehicular Accident, Unprovoked Murder & Assault, Acc. Death)
- Affidavit of Accident (if necessary)
- Medical Certificate (if Form B of PA Claim Form is not accomplished)
B. Death Claim
- Death Certificate
- Birth Certificate
- Certified True Copy of Marriage Certificate (if applicable)
- Certified True Copy of Birth Certificate of Children (if applicable)
- Admitting History, operating room record, discharge summary & other Diagnostic/Laboratory Results
- Autopsy Report (if necessary)
C. Disability/Disablement Claim
- Accomplished Disability Form
- MRI/CT Scan Result (if applicable or necessary)
- Hospital Admitting History and Physical Examination
- Certified Clinical Summary
Step 3:
Kapag aprubado ang iyong claim matatanggap ang iyong claim benefit sa loob ng 8 days.
